McLean High School COVID-19 Health Questionnaire

All visitors must have a scheduled appointment with a staff member to enter the building.  Visitors with appointments, must call the main office 703-714-5700 to check in and must complete the health questionnaire below before being granted access.  

If any visitor answers “YES” to any question on the health questionnaire, then he/she will not be permitted to enter the building.  Visitors who answer “NO” will be instructed to access the building through Door #1 and will be greeted by a masked employee, who will direct them to their meeting.  All visitors must wear a mask while inside the building.

As we continue to practice social distancing, we will limit the number of visitors to our building.  If you have an appointment with a staff member, only bring others with you who are relevant to the meeting. 

We encourage parents and students to email questions or concerns using the staff directory.

All visitors to McLean High School must complete and submit the following COVID-19 health questionnaire prior to entering the building. 

Do you have a fever of 100.4(F) or higher?
Have you used a fever reducing medication within the last 3 days?
Do you have a cough that is not due to another health condition?
Do you have shortness of breath or difficulty breathing that is not due to another health condition?
Do you have chills that are not due to another health condition?
Do you have a sore throat that is not due to another health condition?
Do you have muscle aches that are not due to another health condition or attributed to a specific activity (such as physical exercise)?
Do you have loss of taste or smell?
Have you tested positive for COVID-19 within the past 10 days?
In the past 14 days, have you had close contact (within 6ft for 15 minutes or more) with someone with a suspected or confirmed case of COVID-19?
